Pursuant to the Ordinance on the conditions, organization and manner of practising telemedicine, an application for obtaining a telemedicine centre licence to practice is submitted on a form prescribed by the Croatian Institute of Telemedicine by 1 June of the calendar year at the latest in writing by mail or in person at the address of the Croatian Institute of Telemedicine.

The following documents should be attached to the application:

  1. Decision of the Ministry regarding the fulfilment of conditions imposed for premises, employees, and medical and technical equipment,
  2. Extract from the Commercial Court Registry which shows the information about the company name, seat and activity (the company activity has to be health activity) not older than 6 months on the day of submitting the application,
  3. Proof of havingat disposalpremises where telemedicine activity can be performed (proof of ownership or the right to use),
  4. Certificate, not older than three months, that no criminal proceedings have been instituted against the responsible person in a legal entity,
  5. Proof of paid fee for the procedure of issuing a telemedicine centre licence to practice,
  6. Certificate and technical documentation for programme support, medical diagnostic equipment, computer and communication equipment and infrastructure, and other equipment.

Any incomplete or incorrectly completed application forms will be returned to the applicant for completion or correction. The deadline for completion and/or correction is 15 days. If the applicant does not remedy the deficiencies within the mentioned deadline, the application will be rejected.

Based on the received application, the Croatian Institute of Telemedicine, in cooperation with the applicant, determines whether the conditions have been met with respect to technical facilities, functionality and security of the information system, programme support, medical and diagnostic equipment, computer and information and communication equipment and infrastructure, other equipment and premises intended for the telemedicine centre.

The telemedicine centre licence to practice (hereinafter the licence) is given for a period of four years.
